The Challenge
As part of my first cooperative experience, at Neil Squire Society. I completely redesigned a disability self assessment tool created with PHP, HTML, CSS and SQL.
The Project
The tool was designed to help persons with learning disabilities gauge the severity of their disability. This was accomplished by having the user answer some simple questions about their disability. Their results were then tabulated into a series of written suggestions to help improve their quality of learning.
My Role
I redesigned all pages from the ground up, choosing new colors, fonts and images. I fixed various other UX related issues such as alignment and sizing. I optimized the site for viewing on various mobile and tablet interfaces.
Date
December 2016 to March 2017
Login View
When the web application is first opened, users are met with the login and create account screens. Here users can login to their existing accounts using a username and password or they can create a new one.

Home View
After logging in, users are moved to the home view. Here, depending on the type of user teacher or student, users can access different areas of the site. Teachers can view student reports as well as take the questionnaire. Students can only take the questionnaire.

Results View
Finally, users can view their results in a table. The results are scaled using 0 to 5. The results are then tabulated into a colour coded bar graph so users can easily view their results.

Self Reflection
During this cooperative work term, I learned how to work quickly and efficiently mainly in a individual capacity. Furthermore, I learned how to create a database from the ground up using PHP and SQL. I was able to seek assistance from my co workers when needed. Such skills are also transferable to various other projects and settings as these skills are essential aspects of web development.
